Log into MyChart
MyChart is an online tool that connects you to your personal health information – at home, on the road or at work. We are pleased to offer MyChart at several of our physician practices, with many more launching in 2012. Soon your physicians and your hospitals will share one patient record for your safety and convenience.
With MyChart, at your convenience, you can remotely and securely:
- E-mail your care team directly
- Request prescription renewals
- Request appointments
- View laboratory test results
- View your recent clinic visits
- Access personalized medical education

All of our practices will soon offer patient’s access to their medical record online and this list will be updated regularly.
