Survival Skills Program
| Survival Skills is a three-hour group program designed to provide basic education details on managing diabetes. |
This program is for patients who cannot attend the comprehensive program or who need guidelines quickly before attending the comprehensive program.
The program includes:
- Explanation of diabetes and the types
- Nutrition education and personalized meal plan
- Blood glucose monitoring and American Diabetes Association glucose control guideline
- Medication information
- Acute and chronic complications (detection, treatment and prevention)
- Exercise
- Foot care
- Sick-day guidelines
- Coping with diabetes
- Goal-setting
A three-month and nine-month follow-up is provided for each participant.
